Your child's health information belongs to your family. This notice explains, in plain language, how we use and protect it — and what you can ask us to do with it.
We collect the information needed to care for your child: contact details, medical and dental history, treatment records, x-rays and photographs, insurance details, and billing records. This page is maintained by Southwest Pediatric Dentistry and describes our own practices.
We use health information for treatment, to obtain payment, and for routine practice operations such as scheduling, recall reminders, and quality review. We may share information with other providers involved in your child's care, and with your dental benefit plan for claims.
Uses beyond treatment, payment, and operations — for example marketing, or sharing photographs of your child — are made only with your written authorization, and you may revoke that authorization at any time in writing.
You may request a copy of your child's records, ask us to correct information you believe is inaccurate, request an accounting of certain disclosures, ask us to communicate with you at an alternate address or number, and request restrictions on certain uses. Requests can be made in writing at the front desk.
Records are kept in our practice management system with individual staff logins, and paper documents are stored securely in the office. Our team is trained on confidentiality, and access is limited to the people who need it to do their work.
